I’ve spent my share of time not only visiting and photographing plenty of old burial grounds, but also doing lots of research on historical facts, spiritual beliefs and superstitions regarding them, and for a bit have been wanting to make a post compiling some of the more interesting tidbits.

Cemetery vs. Graveyard: Simply put, a graveyard is located on the same premises as a church, and a cemetery is not, but a seperate plot of land, either across the street from the church, across town, or may be owned by the town rather than a single place of worship.

Gravestone vs. Headstone vs. Tombstone: All three are different, and all hearken back to Jewish customs as far as history knows, tracing back to the first “official” gravemarker that started the tradition (not including wooden crosses), due to a Jewish man; a detective type, who worked on a case even on the Sabbath. The case was closed due to his overwork, but he never forgave himself and requested that he even be “stoned” after death, supposedly resulting in the first documented gravestone. 
      A gravestone, or slab, is a flat horizontal marker. And theoretically protected the body from graverobbing more than the other types, as these things (like most of them) weigh a ton, and graverobbers would have to dig around and then under them to get to the remains, and avoid the slab falling in on them or pinning a limb. 
     Headstones are your run of the mill vertical grave marker, simple, square or rounded with not much detail usually.
     Tombstones are your headstones that are very fancy, featuring carved portraits, death masks, statues, celtic-style crosses, etc.. Basically, the fancier it looks, the more likely its a tombstone rather than a headstone.
          Tombchests are lumped in with Tombstones, your casket sized boxes of stone above where the body is buried. Also called a false-sarcophagus as no body is inside, another deterrant to graverobbers who would waste time and effort breaking in, only to find it hollow, and THEN having to dig under it, and again like a slab hoping it doesn’t fall in on them or pin them. 

Head/Tombstone Facing: Just before anything else is said, there’s no for sure way for a simple visitor to know the reasons, too many variables, but with enough research into the individuals buried, the religion, the church’s customs, and time era and local superstitions… maybe.
     Simply put, a lot of Christians are buried with either their stone facing the rising sun in the East, or, the entrances of the burial area so that when the day comes they rise, they’re already facing the right direction. In addition, some clergy of the church may be buried facing away from those directions, like entrances, so when they rise they will already be facing their flock.
     Other religions may bury so that the stone faces the direction of Jerusalem or Mecca. 
     Finally, the most fun ones are local traditions, in that a popular one is people buried facing East were thought to be good wholesome people and great members of society. While less reputable ones, like theives, known liars, prostitutes, etc.. were buried facing North to South. And finally, evil people and those accused of witchcraft were buried facing the West to be denied any future sunrises.

Superstitions: There’s a few fun ones, such as flowers only grow upon the graves of good people, and any that vines and weeds crawl upon and cling to were bad people. Of course the “it’s bad luck to walk over a person’s grave”, but honestly, there’s no way to traverse hardly any graveyards/cemeteries without doing this, lol. Finally, Iron Crosses are fun, as in elaborate metal crosses in place of a head or tombstone; they’re meant to keep the spirit of the person beneath trapped beneath with the body due to them being feared as evil by the people, trapped so they could not haunt a location or be able to go to their respective afterlife.

There’s plenty more customs and traditions for sure, like the metal cages to prevent undead, and the bells on tombstones in case people were buried alive, and many others, but for the most part this about covers most of what you’d find in American graveyards. I’m sure there’s more tidbits I haven’t learned yet but this is my “so far” compilation. :)
